Targeted Case Management
People living with serious and persistent mental illnesses live fuller lives and have enhanced chances of recovering when they have community services and supports that increase their resilience. At WES, the Targeted Case Management (TCM) program services Philadelphians by assessing their needs and connecting them to the necessary community supports.
WES’s TCM program is at the forefront of similar providers when it comes to creating solutions that assist consumers in reaching a recovery level that no longer requires the aid of case managers. In fact, WES was one of only four behavioral health providers that were initially selected to implement flexible hours to extend the service into the evening (and also affords after-hour emergency contacts). Boasting the lowest turnover rate of TCM programs, WES has very experienced staff whose members are part of the fabric of the communities in which they serve.
WES’s TCM program is also staffed by a forensic specialist and a drug and alcohol specialist who are in-home resources for the case managers.
Adult Partial
The Adult Partial program is located at 2514 North Broad Street on the second floor. The program is open six days a week: Monday through Friday from the hours of 9:00 am to 4:00 pm. and Saturday 9:00 am to 3:00 pm. Persons in Recovery are diagnosed with Schizophrenic, Bipolar, or major depression. The goal is to aid the Person in Recovery to become a functional member of his or her community.
Person in Recovery learn the early warning signs of their illness to prevent relapse. Person in Recovery learn how to become proactive in seeking help with aid of a therapist. The Staff is comprised of two Psychiatrists, Therapists, a Supervisor and Director. Person in recovery have the option of attending groups three to five days a week. On Saturdays Person in recovery attend events throughout their community. Public transportation is encouraged where possible.
We serve an average of 78 Persons in Recovery a day. Treatment Plans are developed with the Person in Recovery and the therapist. On-on–on sessions take place twice a month and more if necessary. The Program has a Psychiatrist available to see a Person in Recovery four days a week.
Each day begins with Community Meting where Person in Recovery shares with their peers such as current events, their goals for the day and what they have learned about their illness. This is a time to also share a talent with the group. The Community meeting is facilitated by a client with a staff person present.
The major groups focus on creating goals, decision making, discharge planning, family issues, medication education, symptoms management, diet and exercise, nutrition, hygiene, problem solving, and arts and craft. There is a library for the clients and some sessions are conducted there. On Fridays the major theme is socializing with each other and learning different ways to interact.
We believe in a holistic way of treating the client and in some cases case conferences are necessary for the client. A team consisting of the Person in Recovery, family members, boarding home provider, Psychiatrist, therapist, and the supervisor or Director meet to see how as a team we can help the client reach his or her goals.
Some services provided are help with obtaining housing where boarding home or individual living arrangements. Person in Recovery interested in developing job skills are connected with agencies that can train and help him or her find a job.
There is a canteen on the unit where Person in Recovery can purchase items at a low price. The Canteen is run by peers. Person in Recovery help to collect the money and decide as a group what should be sold. In this environment the Person in Recovery develops leadership skills as well as being a volunteer. For some, this is the starting point in developing skills to work.
Lunch is served daily.
As part of therapy Person in Recovery attend Field trips to widen their scope of positive things to do in the community. Person in Recovery help decide trips to attend and work with the therapist in the developing process. Each event allows different Person in Recovery to work with staff to providing more the possibility to be a part of the process.
Social Rehabilitation
Groups are very similar to Adult Partial but the difference is in insurance coverage. Person in Recovery can be self pay. The average age of the Person in Recovery ranges from the late thirties to sixties. Although all are diagnosed with a major illness, or depression another common theme is the lack of Socialization skills. Group size is eleven Person in Recovery to one therapist.
MEDICARE PARTIAL
Clients with Schizophrenia, Bipolar, or major depression who are at risk of returning to the hospital have intense therapy to avoid being hospitalized in the Medicare Partial program. Clients are seen daily by the Psychiatrist for 45 minutes and for three hours with a therapist. Clients meet in group session and one on one throughout the day. Attendance is mandatory four days a week. Clients who become stable are then discharged to attend Social Rehabilitation, Partial Hospitalization or to the AOP. In AOP, the client can just come in to see the therapist as needed or just for medication.
